In a statement to the BBC, the publicist said the couple "are taking time apart" in order to "evaluate and work on their marriage".
Douglas, 68, and 43-year-old Zeta-Jones have been married for 13 years.
They first met in France in 1998, before tying the knot in November 2000. They have two children together – Dylan, 13, and Carys, 10.
Rumours that the couple's relationship was in trouble first emerged earlier this year in February, but they subsequently denied they were going through a trail separation. Both parties have struggled with health problems in recent years.
In 2010, Douglas was diagnosed with throat cancer, while Zeta Jones sought treatment for bipolar disorder in 2011 and again earlier this year.
